Center of Excellence – Post-Acute Care

The Vapotherm Center of Excellence (COE) Program recognizes those health care facilities that best use Vapotherm technology to improve patient care and outcomes while maximizing the effectiveness and efficiency of facility staff and resources. Vapotherm COE partners are committed to excellence in their own facilities, and to sharing their knowledge, best practices and experience to improve the standard of care for all patients.

Post-Acute Care facilities provide longer-term medical treatment and/or rehabilitation for patients with chronic illness or complex medical conditions. Post-Acute Care providers include Long Term Care Hospitals (LTACHs), Inpatient Rehabilitation Facilities (IRF) and Skilled Nursing Facilities (SNFs). Vapotherm recently awarded its first Post-Acute Care Center of Excellence designation to Gaylord Hospital, a 137 bed LTACH in Wallingford, CT. Since standardizing on Vapotherm for tracheotomy humidification, Gaylord has experienced meaningful improvement to clinical outcomes and financial results in their ventilator weaning patient population.
